Wellness and Preventive Care
Unifour Family Practice will take the time to ensure that you receive age appropriate wellness and preventive care, such as regular male and female annual physicals and Medicare Wellness Exams. We also encourage vaccine updates to keep our patients fully protected against vaccine preventable disease.
Chronic Medical Management
One of the most important services we provide is the management of chronic medical conditions. We specialize in the management of diabetes, hypertension, high cholesterol, asthma, COPD, thyroid abnormalities, gastrointestinal disorders, anxiety and depression, as well as a host of other common medical conditions. With patient input we work to carefully track medication lists, verify refills, and check for possible drug interactions. We also work closely with other specialists to coordinate care and control costs, especially when transitioning care from an inpatient facility back home.
Comprehensive Women’s Healthcare
We offer the complete spectrum of women’s healthcare services including regular pap and breast exams, regular mammogram and bone density testing through local imaging centers. We also offer management of bladder control issues, menopause and contraceptive management including Nexplanon.
Geriatrics
We are committed to excellent care at all stages of life. We work well with family members who are providing care for elderly parents. We encourage “Welcome to Medicare” and Annual Medicare Physicals. We do memory loss evaluations as well as coordinate in home therapies like speech therapy, occupational therapy, and physical therapy.
When indicated we order handicap parking tags, durable medical equipment such as walkers, canes, lift chairs and scooters. We also coordinate the transition of care from acute care settings like the ER, Urgent Care and the Hospital to longer term settings like rehab facilities and nursing homes, or back to independent living.
Behavioral Health Problems
We recognize that there is a complex interaction between the mind and body. Life can be very stressful and there are times when it can be overwhelming. We commonly diagnose and treat a variety of mood disorders from anxiety to major depression and bipolar depression. We work closely with area psychologists and counselors to provide integrated treatment.
