Minor Consent to Treatment

All minors MUST be accompanied by their Parent and or Legal Guardian on their first visit to our office.  The Legal Guardian complete our parental consent form and our HIPPA form for children. If in the future,  the minor is to be accompanied by someone other than his her Legal Guardian, then these individuals must be listed on the parental  consent form as approved and given permission to authorize treatment on those dates of service.  These forms may be downloaded on our website.

Fees and Insurance

Fees for service (copayments and deductibles) are due and payable at the time of your visit.  For your convenience, we accept cash, checks, Visa, Master Card, and Discovery.  You may also pay your bill on line on our website.  For patients with large deductibles, we can establish a payment plan through Insta Med.  We participate in a variety of insurance plans.  Please bring your insurance card (including Medicare or Medicaid) to each visit.

Refills

In an effort to more accurately and efficiently manage prescription refills, we ask our patients to either request refills using the Patient Portal or by contacting their pharmacy directly. We are no longer able to respond to messages left on our telephone system.

If you do not have internet/email access, then contact your pharmacy to make sure they check for unused refills. They will contact us if new refills are needed.

No Show and Missed Appointments

Our policy is to charge for missed appointments not cancelled within 24 hours preceding the date of your appointment.  These charges will be your responsibility within 24 hours preceding the date of your appointment.
